Individuals of any age should check with their doctor before beginning a fitness program. This questionnaire is designed for people aged 15 to 70.
A client's health history is important to a personal trainer, as it will give insight into potential health concerns, conditions, and medications that may affect their training protocols.
The three-day dietary record form gives clients the opportunity to write down the foods and amounts (if know) that they consume daily.
